Do not send your only copy! It could get lost or damaged. By mailing your print(s) you agree to not hold photocutouts.com responsible for loss or damage. It would be better to take your prints to Kinko's or other such place and have them scan your photo for you. They can even e-mail it for you.
Ask them to clean the scanner glass and wipe any dust off the print. Also make sure the scanner is set for 300 dpi. And ask them to not compress the file.
If your photo was taken by a professional photographer, I'll need you to get a copyright release from him/her. Photographers usually have no problem giving permission for their work to be made into sculptures because they don't offer the product themselves. They have a standard release form so it's not a big deal. I would just need to have the release on file. They can fax it to me at 815-927-0215 or send me an email giving me permission to copy their photo for a photo cutout.
